To help save time at your first visit to the Rheumatology Specialists of Connecticut, please fill out the following 5 forms:

The Patient Registration Form as well as some federally mandated regulatory forms (Consent for Treatment, Advanced Directive, HIPAA Questionaire) helps to provide the practice with some basic information to help begin your treatment.

Also, upon your FIRST  VISIT to Dr. Quan, we will require some basic medical history to help shape the course of treatment.  In order to do so, please fill out our New Patient Intake form. 

 

If your visit is for Osteoporosis, please fill out our Osteoporosis form. 

 

If you are ALREADY a patient of Dr. Quan's, please help us keep track of any changes that may occur in between visits.  In order to do so, please fill out our Follow-up Intake form.

 

If your visit is for Rheumatoid Arthritis,  EVERY VISIT will require you to fill out our quick and easy HAQ Assessment form, which will help us monitor your disease activity.
